2-2A Violation of Code: [511-6-1.03(2)(m)] PIC not discussed with employees their responsibility to report to the PIC information about their health and activities as they relate to diseases that are transmissible through food. COS: Provided information for PIC to go over with employees and employees can sign. Corrected On-Site. New Violation. 2-2D Violation of Code: [511-6-1.07(3)(b)] No paper towels available for drying hands after handwashing at only hand sink in kitchen. Must be able to dry hands after proper handwashing. COS: PIC stocked sink with roll of paper towels. Corrected On-Site. New Violation. 4-1A Violation of Code: [511-6-1.04(4)(c)] Packages of chicken wings laying on packages of bacon and next to package of fully cooked sausage. Raw animal foods must be separated according to minimum cook temperatures by arranging raw animal food requiring the highest internal cook temperature on the lowest shelf and ready-to-eat food items above all raw animal foods. COS: PIC moved chicken to lowest shelf, bacon on shelf above chicken and cooked sausage on shelf above both raw animal foods. Corrected On-Site. New Violation. 6-1A Violation of Code: [511-6-1.04(6)(f)] 2-door sandwich unit not holding time and temperature control for safety foods at required 41F or below. Sliced chees and blue cheese crumbles inside unit held at 52F - 54F. Internal thermometer registering 52F - 54F. Shredded cheese in top of sandwich unit 51F. Sliced tomatoes less than 2 hours 50F on top. COS: PIC voluntarily discarded cheese inside unit. Shredded cheese on top shredded less than 2 hrs, sliced tomatoes, lettuce, shredded cabbage prepped less than 2 hours put in walk-in cooler. Corrected On-Site. New Violation. 6-2 Violation of Code: [511-6-1.04(6)(g)] Many unmarked bags, containers of cooked meat in upright reach-in cooler without date marking. Time and temperature control for safety foods that are made in house and held over 2h hours must be labeled with day or date food item must e consumed on premises, sold or discarded not to exceed 7 days with day of preparation counted as day 1. New Violation. 12C Violation of Code: [511-6-1.04(4)(m)] Many wiping cloths laying on preparation surfaces at start of inspection not being stored in sanitizer between uses. Cloths used to wipe food contact surfaces should be stored in container of chemical sanitizer between uses. COS: Bucket of sanitizer set-up with 200ppm Quat by PIC. Must post most recent inspection 15 feet from main entrance, 5- 7 feet off the floor and in a location that allows customers to view from 1 foot away. Inside of hood needs to be cleaned frequently enough to prevent accumulation of grease that can drip onto food/food contact surfaces. Outside of cabinets under tea urns/drink dispensers and inside cabinets with bag-in-a box need cleaning - sticky outside and accumulation of wood shavings inside from installation of bag-in-a box. New Violation. 17B Violation of Code: [511-6-1.06(5)(o),(p)] Dumpster area with accumulation of leaves and construction trash. keep this area clean to prevent attraction for insects/rodents - harborage area. New Violation. 17C Violation of Code: [511-6-1.07(5)(a),(b)] Floor with water and grease accumulation behind 3-compartment sink, grease trap and under behind water heater. Accumulation of grease/water can be attraction for insects/rodents - keep floors clean and dry. New Violation. 18 Violation of Code: [511-6-1.07(2)(m)] Back door propped open during working hours in the kitchen. Back door may be left open if screen door is installed to control possible insect/rodent entry. If no screen door, door must be kept shut to prevent possible entry of insects/rodents.
