4-1A Violation of Code: [511-6-1.04(4)(c)] Eggs stored above produce and beside avocados and cooked beans in walk-in cooler. Raw shrimp stored beside cheese and above guacamole and chipotle sauce in reach-in cooler. Raw chicken stored above raw steak and chipotle sauce in reach-in cooler. Keep raw animal foods stored spatially separated or below ready-to-eat foods in descending order of lowest to highest required cook temperature. COS- Foods rearranged accordingly. HA furnished food storage poster. Corrected On-Site. New Violation. 6-2 Violation of Code: [511-6-1.04(6)(g)] Many containers of ready-to-eat foods prepared in-house have no date labels (bulk pico de gallo, bulk diced tomatoes, 16 quart salsa, 16 quart chicken soup, bulk chipotle sauce, diced octopus, and tamales). Prepared foods kept more than 24 hours must be date marked and discarded within a maximum of 7 days. COS- foods back-dated to original prep date or voluntarily discarded if prep date unknown. Corrected On-Site. New Violation. 6-2 Violation of Code: [511-6-1.04(6)(h)] Ranchero sauce and salsa stored in reach-in cooler between 9-11 days from date of preparation. Prepared foods cannot be served after a maximum of 7 days from preparation. COS- CFSM voluntarily discarded. Corrected On-Site. New Violation. 12A Thick layers of non-potable water/ice crystals forming on food containers and boxes in walk-in cooler. Non-potable water/ice dripping from behind evaporator fan and collecting on the tops of food containers and boxes. Foods must be stored protected from exposure to non-potable water. Repeat Violation. 12A Violation of Code: [511-6-1.04(4)(i)] Foods held on ice in walk-in cooler stored in containers that do allow for the ice to melt away so product remains in direct contact with undrained water/ice. COS- CFSM placed foods in colander style pan fitted over deep pan which allowed ice to melt off of products. Corrected On-Site. New Violation. 15A Violation of Code: [511-6-1.05(6)(a)] Handle to reach-in cooler broken and styrofoam insulation exposed and collecting accumulated debris and residues. Handle needs replaced so that material surfaces can receive proper cleaning- recommend obtaining a replacement handle designed for specific fit cooling unit. Gasket at walk-in freezer door needs replaced and door serviced so seals tightly when closed to prevent the accumulation of frost on stored foods. New Violation.
