12B Violation of Code: [511-6-1.03(5)(i)] Food service employee working with food at pizza line not wearing hair restraint. Employees preparing or handling food must use effective and clean, disposable or easily cleanable nets or other hair restraints (hats, visors, etc.), worn properly to restrain loose hair including beards and mustaches longer than one half inch. COS: Employee retrieved a hat from his car and put it on Corrected On-Site. New Violation. Remarks CFSM Exp 3/27/2021
