1-2A Violation of Code: [511-6-1.03(2)] No employees on shift assigned the role of Person In Charge (PIC) for overseeing compliance of food safety practices. There must be a PIC on the premises at all times. New Violation. 1-2B Violation of Code: [511-6-1.03(3)(b)] .03(3)(d) It is the responsibility of the Certified Food Safety Manager (CFSM) to designate someone to be the PIC when the CFSM is not on the premises. The CFSM must provide adequate training to whoever they designate as the PIC so that the employee is able to fulfill the duties and responsibilities of PIC. An employee called the CFSM and an acting PIC was designated over the phone for the purpose of the inspection. New Violation. 4-1A Violation of Code: [511-6-1.04(4)(j)] Observed accumulated date labels present on the interior (fcs) surfaces of food containers. Labels were soggy, deteriorating, stained, and trapping debris and grime. Portioned and stacked foods in contact with these labels were observed in the flip-top of the makeline cooler. Foods must contact only clean single service items or properly cleaned and sanitized equipment and utensils. Foods voluntarily discarded by PIC. Corrected On-Site. New Violation. 4-2A Violation of Code: [511-6-1.04(4)(c)1(iv)] Bulk ice bin under front counter left open, boxes of pastries left open and uncovered in walk in cooler and freezer. Bulk Boston Crème dispenser has no lid, cut bulk bags of pumpkin cake and cinnamon/sugar doughnut mix left open on dry storage shelves, bulk rolling containers of doughnut sugars stored without lids. Keep foods covered or secured between uses to protect foods from contamination. Repeat Violation. 4-2B Violation of Code: [511-6-1.05(7)(b)] The makeline cutting board, coffee cream pitcher mixers, and wand-type food contact attachments on beverage machines have accumulated food residues, items cleaned at end of each day per PIC. Surfaces that are in contact with TCS foods must be cleaned and sanitized no less than every 4 hours. Surfaces cleaned/sanitized. Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(7)(a)1] Observed accumulated food debris and/or label residues on clean utensils stacked at air drying rack and throughout food prep areas. Remove residues to sight and touch before sanitizing and storage. Repeat Violation. 12B Violation of Code: [511-6-1.03(5)(g)] Employees handling foods while wearing wrist watches. Food employees must not wear any jewelry (with exception of plain band on finger) our their hands or forearms while preparing foods. Watches removed. Corrected On-Site. New Violation. 12B Violation of Code: [511-6-1.03(5)(i)] Food employee at makeline cooler has long hair down with no restraint. Food employees must keep hair restrained with a hat or hair net when handling foods. Employee left food prep area and secured hair back/donned a visor before returning. Corrected On-Site. Repeat Violation. 12C Violation of Code: [511-6-1.04(4)(m)] No sanitizer registering on test strips for wiping buckets. Bucket solution clouded with debris. Sanitizing cloths caked with debris and either partially submerged in solution or lying out on prep tables and equipment. Keep in-use cloths fully submerged in sanitizing solution between uses and maintain solution at required sanitizer concentration and free of food debris and visible soil. New buckets prepared to 400ppm quat and new cloths stored submerged. Corrected On-Site. Repeat Violation. 15A Violation of Code: [511-6-1.05(6)(a)] Ice accumulated along evaporator fans and attached components in walk-in freezer. Have repaired to prevent non-potable water and ice from accumulating over foods. Repeat Violation. 15C Violation of Code: [511-6-1.05(7)(d)] Slime residue along basins of food prep area handsinks. Accumulation of food debris or spilled/splashed residue on sink ledges, racks, mixing equipment, countertops (especially under equipment), shelving, and exteriors of buck food buckets/containers. Clean surfaces frequently and thoroughly enough to keep rid of accumulations. Repeat Violation. 17B Violation of Code: [511-6-1.06(5)(o),(p)] Boxes and trash on ground in dumpster area. Keep area clean and maintained to prevent pest attraction. Repeat Violation. 17B Violation of Code: [511-6-1.06(5)(n)] Top lid to dumpster left open. Keep closed between uses. PIC closed dumpster lid. Corrected On-Site. New Violation. 17D Violation of Code: [511-6-1.07(4)(b)] Employee personal belongings observed in food prep and storage areas. Employee belongings must be kept in a designated employee break where contamination of food, equipment, utensils and single service items cannot occur. PIC removed items. Corrected On-Site.
