4-2B Violation of Code: [511-6-1.05(7)(b)] Accumulated slime and debris in tea urn nozzle and interior mouth of ice machine chute. Disassemble and clean/sanitize tea nozzles at least every 24 hours and clean/sanitize food contact surfaces of ice machine frequently and thoroughly enough to keep rid of accumulations. COS- CFSM cleaned/sanitized ice machine and tea nozzle. Corrected On-Site. New Violation. 12C Violation of Code: [511-6-1.04(4)(m)] Wet wiping cloths left out on prep table tops or draped over sanitizer spray bottles and sink ledges between uses. Keep wiping cloths stored submerged in sanitizing solution between uses. COS- CFSM placed used cloths in dirty linen and prepared sanitizer buckets to 300ppm quat with new cloths. Corrected On-Site. New Violation. 14A Violation of Code: [511-6-1.04(4)(k)] Scoops for ice machine stored on top machine between uses. Store in-use utensils on surfaces that are adequately kept clean and sanitized. The top of the ice machine is prone to collecting dust, hard to reach for adequate cleaning/sanitizing, and cannot be seen by employees for routine visual inspection. COS- Ice scoops sent through the warewashing machine and placed on clean tray between uses. Corrected On-Site. New Violation. Remarks: - Wash 170, Rinse 196, Surface 183, PSI 30 - Quat Dispenser 300ppm - Discussed and furnished Water Interruption Plan - Facility equipped with emergency 3 day food water supply.
