2-2D - adequate handwashing facilities supplied & accessible 511-6-1.06(2)(o) - using a handwashing sink- operation & maintenance (pf) Points: 4 Corrected during inspection?: Yes Repeat: No Inspector Notes: Observed in-use wiping cloth to be in the hand sink by the 3-compartment sink. 511-6-1.06(2)(o) - Using a Handwashing Sink- Operation & maintenance (Pf) A handwashing sink shall be maintained so that it is accessible at all times for employee use. A handwashing facility may not be used for purposes other than handwashing. COS: The PIC removed the in-use wiping cloth from the hand sink immediately. 2-2E - response procedures for vomiting & diarrheal events 511-6-1.03(6) - clean-up of vomiting and diarrheal events(pf) Points: 4 Corrected during inspection?: No Repeat: No Inspector Notes: Observed without a chemical for use against Norovirus. The facility is using a chemical that is not on the EPA G list. 511-6-1.03(6) - Clean-up of Vomiting and Diarrheal Events(Pf) (6) Responding To Contamination Events. A food establishment shall have written procedures for employees to follow and an EPA registered disinfectant effective against Norovirus available on-site at the food service establishment when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter. Note: The PIC will need to obtain a chemical disinfectant that is approved and on the EPA G list. Correct by: 8/15/2025. 4-2B - food-contact surfaces: cleaned & sanitized 511-6-1.05(6)(n) - manual and mechanical warewashing equipment, chemical sanitization-temperature, ph, concentration, hardness (p,pf) Points: 4 Corrected during inspection?: No Repeat: No Inspector Notes: Observed chlorine sanitizer in 3-compartment sink with a chlorine concentration of 10ppm. 511-6-1.05(6)(n) - Manual and Mechanical Warewashing Equipment, Chemical Sanitization-Temperature, pH, Concentration, Hardness. Chlorine concentration should be between 50-100ppm. Note: The PIC will need to obtain a new chlorine sanitizer that is approved by the EPA. Correct by: 8/8/2025 (72 hours). 14A - in-use utensils: properly stored 511-6-1.04(4)(k) - in-use utensils, between-use storage (c) Points: 1 Corrected during inspection?: Yes Repeat: Yes Inspector Notes: Observed ice scoop with handle in contact with ice in the reach-in freezer. 511-6-1.04(4)(k) - In-Use Utensils, Between-Use Storage (C) (k)Storage of In-Use Utensils. During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored in food that is not time/temperature control for safety food with their handles above the top of the food within containers or equipment that can be closed, such as bins of sugar, flour, or cinnamon. COS: The PIC immediately placed the scoop in a different area in the freezer. 16B - plumbing installed; proper backflow devices 511-6-1.06(2)(a) - plumbing system, approved (p) Points: 2 Corrected during inspection?: No Repeat: No Inspector Notes: Observed a residential water filter connected to the 3-compartment sink being used for herbalife smoothies that is not NSF/ANSI approved. 511-6-1.06(2)(a) - Plumbing System, Approved. A water filter shall be made of safe materials. Pf Note: The PIC will need to remove the water filtration system and submit proof of NSF/ANSI certification for a water filtration system.
