2-2B - proper eating, tasting, drinking, or tobacco use 511-6-1.03(5)(k)1&2 - eating, drinking, or using tobacco (c) Points: 4 Corrected during inspection?: Yes Repeat: No Inspector Notes: Observed Gatorade bottle stored outside of "employee food" container in the walk-in cooler. Employees shall consume food only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas. However, drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee’s hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles will be allowed. Corrective Actions: PIC immediately placed Gatorade bottle in designated "employee food" container. 12B - personal cleanliness 511-6-1.03(5)(i) - hair restraints (c) Points: 3 Corrected during inspection?: Yes Repeat: Yes Inspector Notes: Observed food employee with a beard longer than 1/2 inch actively prepping food without a bear restraint. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. Corrective Actions: PIC instructed employee to obtain beard restraint. 15B - warewashing facilities: installed, maintained, used; test strips 511-6-1.05(3)(h),(i),(j) - temperature measuring device, manual warewashing; sanitizing solutions, testing device (pf) Points: 1 Corrected during inspection?: No Repeat: No Inspector Notes: No test strips available for QAC sanitizing solution. Sanitizing Solutions, Testing Devices. A test kit or other device that accurately measures the concentration in mg/L or ppm of sanitizing solutions shall be provided. Corrective Actions: EHS tested QAC sanitizing solution concentration and observed to be in compliance. PIC stated new test strips will be ordered on next truck order. 15C - nonfood-contact surfaces clean 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c) Points: 1 Corrected during inspection?: No Repeat: No Inspector Notes: Observed food debris and dust accumulation on floors, walls, and ceilings and equipment throughout kitchen. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Corrective Actions: PIC stated nonfood contact surfaces willbe cleaned. 17A - toilet facilities: properly constructed, supplied, cleaned 511-6-1.06(5)(h) - toilet room receptacle, covered (c) Points: 1 Corrected during inspection?: No Repeat: No Inspector Notes: Observed trash can in unisex restroom without a cover. Toilet Room Receptacle, Covered. A toilet room used by females shall be provided with a covered receptacle for sanitary napkins. Corrective Actions: PIC stated a lidded trash can will be obatined for the restroom. 17C - physical facilities installed, maintained, and clean 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c) Points: 1 Corrected during inspection?: No Repeat: No Inspector Notes: Observed fan in main kitchen and ventilation in restroom to have heavy dust accumulation. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. Corrective Actions: PIC stated all dust accumulation will be cleaned.
