1-2A - pic present, demonstrates knowledge, performs duties 511-6-1.03(2)(a)-(n)(p),(q) - responsibility of pic (pf) Points: 4 Corrected during inspection?: No Repeat: No Inspector Notes: Observed no temperatures recorded on most recent key drop delivery invoice (5/19/2025) from U.S. Foods. PIC (person in charge) was not able to produce key drop agreement from U.S. Foods. Monitoring of Receiving during Non-Operating Hours. Employees are verifying that foods delivered to the food service establishment during non-operating hours are from approved sources and are placed into appropriate storage locations such that they are maintained at the required temperatures, protected from contamination, unadulterated, accurately presented, by routinely monitoring the employees' observations, maintaining receiving/corrective action records for deliveries during non-operating hours and periodically evaluating foods upon their receipt as specified within DPH Rule 511-6-1-.04(3)(m). Corrective Actions: PIC stated U.S. Foods will be contacted to obtain key drop agreement and truck drivers will be notified of their responsibility to record temperatures of at least 2 TCS (time/temperature controlled for safety) food items during key drop deliveries. 4-2B - food-contact surfaces: cleaned & sanitized 511-6-1.05(6)(n) - manual and mechanical warewashing equipment, chemical sanitization-temperature, ph, concentration, hardness (p,pf) Points: 4 Corrected during inspection?: Yes Repeat: No Inspector Notes: Observed QAC (quaternary ammonium compounds) sanitizing solution to have temperature above 75°F per manufacturer specifications. A quaternary ammonium compound solution shall have a minimum temperature of 75ºF (24ºC), have a concentration as specified in DPH Rule 511-6-1-.07(6)(g) and as indicated by the manufacturer’s use directions included in the labeling, and be used only in water with 500 ppm hardness or less or in water having a hardness no greater than specified by the EPA-registered label use instructions. Corrective Actions: PIC (person in charge) made new QAC sanitizing solution at proper temperature per manufacturer specifications. 4-2B - food-contact surfaces: cleaned & sanitized 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf) Points: 4 Corrected during inspection?: Yes Repeat: No Inspector Notes: Observed encrusted material on can opener blade. Equipment food-contact surfaces and utensils shall be clean to sight and touch. Corrective Actions: PIC (person in charge) cleaned can opener blade of encrusted material. 13A - posted: permit/inspection/choking poster/handwashing 511-6-1.02(1)(d) - displaying of the inspection report (c) Points: 1 Corrected during inspection?: No Repeat: No Inspector Notes: The top 1/3 of the most current inspection report was not posted in the drive-thru window. Food service establishments with drive-thru windows will post the current inspection report, and also have the inspection report posted so that a minimum of the top one-third of a copy of the current inspection report is visible through each window allowing customers to easily read the score, date of inspection and establishment information. Corrective Actions: PIC (person in charge) stated today's inspection report will be copied and posted in drive-thru window. 15B - warewashing facilities: installed, maintained, used; test strips 511-6-1.05(3)(h),(i),(j) - temperature measuring device, manual warewashing; sanitizing solutions, testing device (pf) Points: 1 Corrected during inspection?: No Repeat: No Inspector Notes: Observed no QAC (quaternary ammonium compounds) sanitizer test strips available at time of inspection. A test kit or other device that accurately measures the concentration in mg/L of sanitizing solutions shall be provided. Corrective Actions: PIC (person in charge) stated QAC test strips will be obtained to ensure proper sanitizing solution concentration. 15C - nonfood-contact surfaces clean 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c) Points: 1 Corrected during inspection?: No Repeat: No Inspector Notes: Observed gasket to reach-in freezer that stores frozen french fries to have accumulation of black debris. Observed gasket to walk-in cooler to have accumulation of black debris. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Corrective Actions: PIC (person in charge) stated gaskets will be cleaned at COB (close of business) and will be added to regular cleaning schedule to prevent heavy accumulation. 17A - toilet facilities: properly constructed, supplied, cleaned 511-6-1.07(2)(l) - toilet rooms, enclosed (c) Points: 1 Corrected during inspection?: No Repeat: No Inspector Notes: Observed both doors that lead into unisex restrooms not self-closing. Except where a toilet room is located outside a food service establishment and does not open directly into the food service establishment, such as a toilet room that is provided by the management of a shopping mall, a toilet room located on the premises shall be completely enclosed and provided with a tight-fitting and self-closing door. Corrective Actions: PIC (person in charge) stated maintenance will be contacted to install self-closing devices on both unisex restroom doors. 17B - garbage/refuse properly disposed; facilities maintained 511-6-1.06(5)(n) - covering receptacles (c) Points: 1 Corrected during inspection?: No Repeat: No Inspector Notes: Observed half of lid on shared dumpster to be in disrepair and other half to be missing. Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight-fitting lids or doors if kept outside the food service establishment. Corrective Actions: PIC (person in charge) stated appropriate personnel will be contacted to have lid(s) on dumpster replaced. 17C - physical facilities installed, maintained, and clean 511-6-1.07(5)(f) - drying mops (c) Points: 1 Corrected during inspection?: Yes Repeat: No Inspector Notes: Observed 2 mops being stored directly on floor and inside bucket. After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies. Corrective Actions: PIC (person in charge) placed mop on wall mount to allow air-drying.
