12B - personal cleanliness 511-6-1.03(5)(i) - hair restraints (c) Points: 3 Corrected during inspection?: Yes Repeat: No Inspector Notes: Upon entry into the food establishment, an employee was observed to not have on an approved hair restraint while actively preparing food. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. COS: Employee voluntarily placed a hair net on during the inspection. 12B - personal cleanliness 511-6-1.03(5)(g) - jewelry (c) Points: 3 Corrected during inspection?: Yes Repeat: No Inspector Notes: Observed and employee to have on a ring while actively preparing food. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. COS: Employee voluntarily removed the ring during the inspection. 15C - nonfood-contact surfaces clean 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c) Points: 1 Corrected during inspection?: No Repeat: Yes Inspector Notes: Accumulated dust, debris, mold and/or residues observed on interiors/exteriors of coolers and cooking equipment, condenser fan covers, and non-food contact portions of table-top food preparation equipment/utensils, along the floors and walls, and along shelving in the walk-in cooler. Kitchen equipment and surfaces need deep cleaned and maintained through routine cleaning frequent and thorough enough to keep surfaces rid of accumulated dust and residues.
