Violation of Code: [511-6-1.07(3)(b)] Observed hand sink near meat prep paper towel dispenser not dispensing paper towels. Each handwashing sink or group of adjacent handwashing sinks shall be provided with: 1. Individual, disposable towels; 2. A continuous towel system that supplies the user with a clean towel; Note: PIC will ensure paper towels are provided at respective hand sink. New Violation. Correct By: 06/27/2024 2-2D Violation of Code: [511-6-1.06(2)(o)] Observed employee rinse knife in hand sink. A handwashing facility may not be used for purposes other than handwashing. COS: Employee educated on proper use of hand sinks. Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(7)(a)1] Observed accumulation of mold-like substance on interior of ice machine. Observed old labels stuck to food containers after cleaning. Equipment food-contact surfaces and utensils shall be clean to sight and touch. Note: PIC will increase cleaning frequency of bulk ice machine. PIC will ensure all old labels stuck to food containers are completely removed. **Second consecutive violation** Repeat Violation. Correct By: 06/27/2024 4-2B Violation of Code: [511-6-1.05(6)(n)] Observed QAC solution with concentration of 100 ppm. QAC solution shall have minimum a concentration of 200 ppm. Note: PIC replaced QAC solution with fresh solution at proper concentration of 300 ppm. Corrected On-Site. New Violation. 15B Violation of Code: [511-6-1.05(2)(y),(z),(aa),(bb)] Observed inaccurate/damaged gauges on dishmachine. A warewashing machine shall be equipped with a temperature measuring device that indicates the temperature of the water: 1. In each wash and rinse tank; and 2. As the water enters the hot water sanitizing final rinse manifold or in the chemical sanitizing solution tank. Temperature measuring device shall be maintained in good condition. Note: PIC informed EHS that Ecolab is scheduled to service dishmachine by the end of the month. New Violation. Correct By: 06/27/2024
