No copy of the CFSM was kept on record in the facility. The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority. Note: PIC will post CFSM of a supervisory employee. PIC will take a CFSM course if no CFSM is available. 4-1A - food separated and protected 511-6-1.04(4)(c)1(i)(ii)(iii)(v)(vi)(vii)(viii) - packaged & unpackaged food separation, packaging, and segregation (p, c) Points: 9 Corrected during inspection?: Yes Repeat: No Inspector Notes: Observed raw fish stored next to and above RTE foods and observed raw chicken next to RTE foods in walk-in cooler. Food shall be protected from cross contamination by: (i) Except as specified in paragraph (i)(III) of this subsection, separating raw animal foods during storage, preparation, holding, and display from: (I) Raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables, P and (II) Cooked ready-to-eat food Note: PIC immediately rearranged walk-in cooler so that raw animal foods were stored properly. 8-2B - toxic substances properly identified, stored, used 511-6-1.07(6)(b) - working containers, common name (pf) Points: 4 Corrected during inspection?: Yes Repeat: No Inspector Notes: Observed multiple bottles of unlabeled chemicals in main kitchen area. Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material. Note: PIC immediately labeled all chemicals 8-2B - toxic substances properly identified, stored, used 511-6-1.07(6)(g) - chemical sanitizers, criteria (p) Points: 4 Corrected during inspection?: Yes Repeat: No Inspector Notes: Observed chlorine sanitizer with a concentration much greater than 100 ppm. A chlorine sanitizer must have a concentration of 50-100 ppm Note: PIC immediately made new sanitizer with a concentration of 100 ppm. 14A - in-use utensils: properly stored 511-6-1.04(4)(k) - in-use utensils, between-use storage (c) Points: 1 Corrected during inspection?: Yes Repeat: No Inspector Notes: Observed multiple containers of non-TCS foods to have scoops either without handles or with their handles touching the food. In food that is not time/temperature control for safety food with their handles above the top of the food within containers or equipment that can be closed, such as bins of sugar, flour, or cinnamon Note: PIC immediately removed scoops without handles and rearranged scoops with handles so that the handles did not touch the food.
