BUFORD—Brookdale Buford, an assisted living facility on Old Hamilton Mill Road in Buford, has failed a routine health inspection.

The facility received a score of 60 (U) during its inspection Thursday morning, according to the report posted on the Georgia Department of Health and Human Services website.

Brookdale, which operates two facilities that also offer Alzheimer's care, was cited for violations that included employees not washing their hands, cleaning chemicals not being stored properly, food-contact surfaces not being properly cleaned and sanitized and eating utensils not being properly stored, dried and handled.

The establishment had received a 97 and 100 on its previous two health inspections, which were held January 14, 2015 and November 13, 2014, respectively.

A follow-up inspection will be held in 10 days.
