Medlen said Sunday the surgery will be performed in Birmingham by Dr. James Andrews. Recovery from the Tommy John surgery generally takes one year.
Medlen sustained the injury on Aug. 4 while pitching against the New York Mets. Doctors waited for swelling to subside before confirming the surgery is necessary.
The second-year pitcher emerged as the team's No. 5 starter and was 6-2 with a 3.68 ERA in 31 games, including 14 starts.
Rookie Mike Minor has replaced Medlen at the end of the rotation.
