ATLANTA - State officials have recommended that a Warner Robins nursing home be fined and have threatened to revoke its license after a patient with Alzheimer's wandered away and was struck and killed.

The Georgia Department of Human Resources determined that the man had wandered away from the Warner Robins Rehabilitation and Nursing Center 17 times prior to Aug. 31, when he was hit by a sport utility vehicle.

The DHR's investigation found that conditions at the nursing home posed ``immediate jeopardy'' to residents' health and safety. Officials cited a lack of patient supervision.

Police said Anthony Weldon, 81, of Warner Robins, was crossing a road around 10 p.m. when he was hit a block from the nursing home.

Center administrator Bill Sampson said staff members searched the premises and notified authorities as soon as they heard the alarm the night Weldon wandered away.

The DHR's Office of Regulatory Services withdrew the nursing home's nurse aide training program, stopped paying for newly admitted patients and recommended federal Medicaid funding be withdrawn by Sept. 29 if the problems are not fixed. The state also said it may withdraw the nursing home's license.

The DHR also recommended a fine of $5,000 a day, dating back to Sept. 6, if federal regulators determine the nursing home has not addressed problems.

The nursing home must submit its plans for correcting the problems to federal regulators by Sept. 19.

Warner Robins Rehabilitation is owned by Kindred Healthcare Inc. of Louisville, Ky.
