Violation of Code: [511-6-1.03(4)(a)(b)(c)(e)(f)] Establishment has no verifiable way to show that food workers have been trained on employee health and reporting illnesses. A food service establishment must train its food workers on when they should report an illness and not report to work. The health authority provided employee health reporting agreements in both English and Spanish to the establishment. CFSM to have all food workers review and sign this form. CFSM to keep the signed forms on site at all times. New Violation. 15A Violation of Code: [511-6-1.05(2)(a)] Observed metal screws used to secure baffle in both ice machine with an accumulation of rust. Food contact surfaces shall be smooth, easily cleanable, and made of durable materials. PIC to replace screws with a durable alternative. New Violation.
