Violation of Code: [511-6-1.05(2)(h)] Observed mold accumulation located on the shelving of 2-door cooler. Nonfood contact surfaces should be cleaned and sanitized at a frequency to prevent mold debris and accumulation. New Violation. 17C Violation of Code: [511-6-1.07(5)(a),(b)] Vinyl floor tiles that have been on floor less than a year with top layer of tile beginning to peel in areas where employees stand frequently to perform duties - in front of grill and in front of hot holding drawer unit and random areas throughout kitchen. Look into future replacement with more durable tiles. Peeling spots can allow water/food to accumulate under top coating of tile which can deteriorate the floor (tile) and not allow for easy cleaning. Repeat Violation. 17C Violation of Code: [511-6-1.07(5)(a),(b)] Observed ceiling leaking water from the roof. Physical facilities should be maintained at a frequency to prevent possible contamination. New Violation.
