Violation of Code: [511-6-1.03(5)(i)] Observed employee involved in food preparation without hair restraint to prevent possible contamination. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. COS: PIC provided proper hair restraints during inspection. Corrected On-Site. New Violation. 16B Violation of Code: [511-6-1.06(2)(r)] Observed 3 Compartment sink in poor working condition. Observed bar sink not properly installed causing a leak. A plumbing system shall be repaired according to law; and maintained in good repair. Facility must correct within (10) days time. New Violation. Correct By: 07/01/2022 17C Violation of Code: [511-6-1.07(5)(a),(b)] Observed multiple ceiling tiles missing in main kitchen area. Physical facilities shall be maintained in good repair. Facility must correct within (10) days time. New Violation. Correct By: 07/01/2022
