Violation of Code: [511-6-1.03(5)(i)] Observed grill employee with no hair net. Employees shall use effective and clean, disposable or easily cleanable net or other hair restraints approved by Health Authority. COS: Employee put on hairnet during inspection. Corrected On-Site. New Violation. 16B Violation of Code: [511-6-1.06(2)(d)] Observed drainage pipe from ice machine to be directly plumbed to the sewer line without an one inch air gap. An air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch (25mm). New Violation. 17C Violation of Code: [511-6-1.07(2)(b)] Observed exposed wires around electrical panel box, near back door and in ceiling due to missing ceiling tiles. Lines may not be unnecessarily exposed. New Violation. 17C Violation of Code: [511-6-1.07(2)(a)] Observed multiple missing ceiling tiles, hole in the floor by dish machine and water leaking above dish machine. The ceiling should be completely covered and all physical facilities shall be maintained in good repair. Repeat Violation. 17D Violation of Code: [511-6-1.05(3)(d)] Observed ceiling tiles to be brown in color and the hood system to not be properly working. Ventilation hood systems and devices shall be sufficient to prevent condensation and grease from collecting on walls and ceilings. New Violation.
