Violation of Code: [511-6-1.04(1)] Observed (6) cans with significant dents, compromising hermetic seal. No designated area was identified for storage of dented cans. Food shall be safe, unadulterated, and honestly presented. COS: PIC designated an area for dented cans and placed signage for easy identification. PIC then placed dented cans in designated area to return to supplier. Corrected On-Site. New Violation. 6-1A Violation of Code: [511-6-1.04(6)(f)] Observed raw shelled eggs cold holding at 46 degrees Fahrenheit, boiled eggs cold holding at 46 degrees Fahrenheit, provolone cheese and milk both cold holding at 46 degrees Fahrenheit in reach in units. TCS foods shall be maintained at 41°F (5°C) or below. COS: Food items identified as properly stored within 4 hours were moved to ice baths and the walk in freezer unit, items with previous temperatures unable to be verified were discarded. Corrected On-Site. New Violation. 6-1C Violation of Code: [511-6-1.04(6)(d)] Observed prepped shrimp and chicken improperly cooling in a reach in cooler and diced tomatoes improperly cooling in a prep top cooler. Cooked time/temperature control for safety food shall be cooled: (i) Within 2 hours from 135°F (57°C) to 70°F (21°C) and (ii) Within a total of 6 hours from 135°F (57°C) to 41°F (5°C) or less. 2. Time/temperature control for safety food shall be cooled within 4 hours to 41°F (5°C) or less if prepared from ingredients at ambient temperature, such as reconstituted foods and canned tuna. COS: During inspection, inspector had PIC move all TCS foods to the walk in freezer and ice baths to cool rapidly. Corrected On-Site. Repeat Violation. 14B Violation of Code: [511-6-1.05(10)(a)] Observed pans stacked before they were air dried and a cloth used to dry dishes. Washed, rinsed, and sanitized utensils and equipment must be stored in a self-draining position that allows air drying and covered or inverted. COS: PIC unstacked pans to allow them to air dry. Corrected On-Site. New Violation. Remarks CFSM Paresh Patel exp. 03/23/2024 Note: Sanitizing buckets and 3 compartment sink sanitizers all with a concentration of 200-400 ppm of Quaternary Ammonium sanitizing solution. Note: Water temperatures- 3 compartment sink 126 degrees Fahrenheit, Employee hand wash sink 120 and 126 degrees Fahrenheit and Restroom 100 degrees Fahrenheit. Note: EPA Reg No. 5813-121 (Clorox Bleach) used as antimicrobial effective against Norovirus. - Discussed cleaning frequency of food contact surfaces, methods for glove changing and hand washing, proper storage of dry good foods and effective methods for food storage order.
