Violation of Code: [511-6-1.07(3)(a)] Observed hand sink nearest to 3-compartment sink to be out of soap. Each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap. COS: PIC replaced empty soap with a new full container. Corrected On-Site. New Violation. 12B Violation of Code: [511-6-1.03(5)(g)] Observed employee actively preparing food with a bracelet on wrist. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. COS: PIC had employee remove bracelet. Corrected On-Site. New Violation. 12B Violation of Code: [511-6-1.03(5)(i)] Observed an employee actively preparing food with no hair restraint. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. COS: PIC provided employee with a hair restraint. Corrected On-Site. New Violation. 15C Violation of Code: [511-6-1.05(7)(a)2,3] Observed bottom of freezer, cabinet where soda tubing is located, and glass front cooler to have an accumulation of food debris. The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. New Violation. Correct By: 04/29/2021
