Violation of Code: [511-6-1.04(6)(f)] Observed cooked shrimp and raw steak holding in cooler at 47F. Cooer was off. PIC states chef bumped plug when cleaning earlier and did not plug cooler back in. Coolers must remain plugged in at all times. COS: Chef plugged cooer in, TCS food cooled appropriately. Corrected On-Site. New Violation. Violation of Code: [511-6-1.04(6)(e)] Observed queso cooling in deep pans in walk-in cooler with lids on. Discussed with PIC cooling queso in shallow pans with lids off stacked across one another in freezer or in ice bath. COS: PIC moved queso to freezer to cool. Corrected On-Site. New Violation. Violation of Code: [511-6-1.05(6)(a)] Observed door of preptop cooler broken off cooler. Observed handles of reach-in cooler broken. All equipment must be maintained in good repair. Have broken equipment repaired immediately. New Violation. Violation of Code: [511-6-1.07(5)(a),(b)] Multiple tiles observed missing next to cooler in grill area and broken tiles at bar area. Holes in tile where men's room urinal and toilet are supplied. Repaired tile ASAP. New Violation. Violation of Code: [511-6-1.07(2)(m)] Observed many flies in kitchen with both back screen doors left open. Screen doors to kitchen must be kept closed when not in use. COS: PIC closed both back screen doors. Corrected On-Site. New Violation.
