Violation of Code: [511-6-1.03(3)(a)] No one employed is currently certified as a food safety manager. Food service establishments shall have in its employ a Certified Food Safety Manager (CFSM) as specified in paragraph (b) of this subsection to ensure food safety is being managed within the food service establishment during all hours of operation as specified within paragraph (d) of this subsection. PIC to notify health authority within 10 days of inspection that they have enrolled in a food safety manager course or of scheduled a test date. New Violation. 18 Violation of Code: [511-6-1.07(5)(o)] When the inspector arrived a dog was observed to be within the food preparation area. Except as specified in paragraphs 2, 3, and 4 of this subsection, live animals may not be allowed on the premises of a food service establishment. COS: PIC took the dog outside of the food service establishment during the inspection. Corrected On-Site. New Violation.
