Violation of Code: [511-6-1.03(3)(c)] Certificate currently posted is for an employee who no longer works at this location. Current manager states she took the CFSM course but does not have the documentation. The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority. 2. A CFSM certificate which has expired, been revoked or suspended shall not be posted in the food service establishment. The establishment shall have 10 calendar days after the inspection, for the permit holder to provide evidence of manager enrollment in a course, a current certificate of a current manager, or hiring of a new manager with a valid CFSM certificate. New Violation. 2-2D Violation of Code: [511-6-1.06(2)(o)] Observed one handwashing sink with a portable cooler sitting in front of it, restricting access, and another with a trach can obstructing access to it. A handwashing sink shall be maintained so that it is accessible at all times for employee use. COS: PIC moved cooler and trash can to allow access to handwashing sinks. Corrected On-Site.
