Violation of Code: [511-6-1.03(2)(m)] Establishment unable to provide verifiable method showing employees have been trained to report food borne illnesses or symptoms of illnesses. Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. COS: Inspector provided establishment with employee reporting agreements to be signed by all employees. Corrected On-Site. New Violation.
