Violation of Code: [511-6-1.07(5)(a),(b)] Observed accumulation of debris on floors, shelves, inside cabinets, around equipment and underneath 3-compartment sink. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. PIC to clean/have employees clean at a frequency which prevents build-up which can attract insects/rodents or contaminate food. New Violation. 17C Violation of Code: [511-6-1.07(5)(a),(b)] Observed sanitizer basket broken off wall and on floor under 3-compartment sink. Observed light bulb burned out above coffee preparation area. All physical facilities shall be maintained in good repair. Management to secure shelving to wall and replace light bulb. New Violation.
