Violation of Code: [511-6-1.04(4)(j)] Observed raw chicken pieces on top of lid of ready-to-eat salsa. Raw animal foods must be stored away from ready-to-eat foods to prevent cross contamination. COS: PIC removed lid and replaced with a clean lid. Corrected On-Site. New Violation. 4-2A Violation of Code: [511-6-1.04(4)(c)1(iv)] Observed sliced onions stored uncovered in walk-in cooler underneath dripping water from walk-in cooler evaporator fan. A piece of tape from a box had been stuck in the evaporator fan drip pan creating a path for water to drip onto food below. Water from the evaporator fan is non-potable water. PIC removed the tape and discarded the onions. Time/temperature controlled for safety foods must be hot held at 135F or higher. COS: PIC reheated queso to 165F within 20 minutes. Corrected On-Site. New Violation. 15A Violation of Code: [511-6-1.05(6)(a)] Observed walk-in cooler metal split at seam and curving upwards. Walk-in cooler floor need to be repaired/replaced with a smooth easily cleanable covering with no seams that allow water underneath. New Violation. 15A Violation of Code: [511-6-1.05(6)(a)] Vent hood suction needs to be adjusted down to prevent grease collection panels from being sucked up into hood.
