Inpection Notes

Violation of Code: [511-6-1.03(2)(m)] PIC could not furnish any documentation verifying that all employees had been trained in reporting illnesses as it relates to diseases that are transmissible through food. PIC to make copies of the posted employee health reporting agreement and have all employees complete one and maintain them on site. New Violation. 8-2B Violation of Code: [511-6-1.07(6)(g)] Observed bleach spray at a concentration much higher than the maximum 100ppm chlorine. COS: PIC and employees were educated on the importance of diluting bleach chemicals to the appropriate concentration for food service use (50-100ppm). Employee remade bleach spray to a concentration of 100ppm. Corrected On-Site. New Violation. 15C Violation of Code: [511-6-1.05(7)(a)2,3] Observed a heavy grease accumulation on light above flat top grill under vent hood. Non-food contact surfaces must be cleaned to prevent food and debris accumulation. PIC to clean light when it is safe to do so. New Violation. 16B Violation of Code: [511-6-1.06(2)(r)] Observed 3 compartment sink faucet and dish machine both leaking when in operation. Plumbing systems must be maintained in good repair. PIC to have plumber fix 3 compartment sink faucet and dish machine. New Violation. 16B Violation of Code: [511-6-1.06(2)(d)] No air gap observed between ice machine plumbing and drain. A one inch air gap must be maintained between the ice machine and the floor drain to prevent potential back flow. PIC to have plumber install a one inch air gap. New Violation. 17B Violation of Code: [511-6-1.06(5)(n)] Observed dumpster in back of establishment with both door left open. Dumpster doors must be kept closed when not in use. PIC to shut dumpster doors immediately. New Violation.
