Violation of Code: [511-6-1.03(3)(b)] Certified Food Safety Manager posted James Weldon is also listed as the CFSM for Subway #14228 at 1220 Thompson Bridge Rd NE Gainesville GA 30501. James Weldon Cert No. 10586210 that is posted expired 12/14/2018. Facility needs a CFSM that is on staff at this location and has supervisory responsibilities at this location alone. Two separate facilities cannot share a CFSM. CFSM certifications must be maintained and current. Facility to enroll an employee with supervisory responsibilities within 30 days. This is an ongoing issue with this Subway location and has been cited on the past 2 inspections making this the 3rd occurrence of issues with maintaining a CFSM. New Violation. 2-2A Violation of Code: [511-6-1.03(2)(m)] PIC was unable to show verifiable means by which employees have been trained on their reporting responsibilities as they relate to foodborne illnesses and their health. PIC was educated on the need to have all employees trained in employee health reporting. PIC to make copies of DPH employee health reporting agreement and have all employees review and sign the form. New Violation. 4-2B Violation of Code: [511-6-1.05(6)(n)] Observed sanitizer bucket and sanitizer in knife cleaning stations at 50ppm QAC sanitizer concentration. QAC sanitizer must be maintained at concentrations according to the manufacturer (usually 150-400ppm). COS employees discarded sanitizer and filled the bucket and knife cleaning stations with sanitizer that measured 200ppm. New Violation. 11C Violation of Code: [511-6-1.04(6)(c)] Observed beef sitting on countertop at 74F. Employee states beef has just finished thawing in running hot water. TCS foods may be thawed as part of the cooking process, submerged completely in cool running water, or under refrigeration at 41F. COS: PIC placed beef in walk-in freezer to cool to 41F before serving. Employees were educated on the need to use cool water when thawing foods. Corrected On-Site. New Violation.
