Violation of Code: [511-6-1.06(2)(c)] Observed handsink in employee restroom not able to provide water at 100F. All handsinks in a food service establishment must be able to provide water at least 100F. Employees to use handsink directly around corner of restroom for handwashing. Handsink in employee restroom to be repaired by 11/17/19. New Violation. 16B Violation of Code: [511-6-1.06(2)(r)] Observed main kitchen handsink leaking underneath bowl. Plumbing systems must be maintained in good repair. PIC to have sink repaired by 11/17/19. New Violation. 16B Violation of Code: [511-6-1.06(2)(d)] Observed 3 compartment sink and vegetable sink plumbing designed without a 1 inch air gap. An air gap must be maintained to prevent potential back siphonage. 1 inch air gap to be installed for vegetable and 3 compartment sink by 11/17/19. New Violation. 17B Violation of Code: [511-6-1.06(5)(o),(p)] Observed dumpster enclosure with an accumulation of grease, trash, and leaf debris on the ground. Areas for storing refuse must be kept clean. PIC to have receptacle enclosure cleaned. Dumpster area to be cleaned by 11/17/19. Repeat Violation. 17C Violation of Code: [511-6-1.07(5)(a),(b)] Observed holes and gaps in FRP paneling on wall to left of main kitchen handsink. FRP paneling should be replaced and holes and gaps filled in. Facilities must be maintained in good repair. FRP to be repaired by 11/17/19. Repeat Violation. 17C Violation of Code: [511-6-1.07(2)(a)] Observed ceiling tile missing above hot water heater and replaced with a piece of loose cardboard. Ceilings should be constructed of smooth, durable, and easily cleanable materials. Ceiling tile to be replaced by 11/17/19. New Violation.
