Violation of Code: [511-6-1.05(7)(a)1] Observed ice machine with a heavy accumulation of slime on the ceiling around chute. Food contact surfaces shall be cleaned and sanitized to sight and touch. PIC to empty ice bin and clean the interior. It is recommended to clean ice machine once/week. New Violation. 6-1A Violation of Code: [511-6-1.04(6)(f)] Observed raw chicken and beef in reach in cooler across from grill at temperatures 50-52F. Time/temperature controlled for safety foods that are cold held must be kept at 41F or lower. COS: PIC states chicken and beef had been placed in reach-in cooler from walk-in cooler at 12:00PM. PIC moved chicken and beef to walk-in cooler to cool. See Log. Corrected On-Site. New Violation. 6-1B Violation of Code: [511-6-1.04(6)(f)] Observed cooked chicken and beef stored on table next to grill in pans at 92F and 101F respectively. Time/temperature controlled for safety foods that are hot held must be maintained at 135F or higher. COS: PIC voluntarily discarded chicken and steak. Corrected On-Site. New Violation. 6-1C Violation of Code: [511-6-1.04(6)(d)] Observed 2 large containers of cooked refried beans labeled 7/24/19 at 52F in walk-in cooler. PIC states beans had been prepared on 7/24/19. Time/temperature controlled for safety foods must be cooled to 70F within 2 hours and to 41F within 4 hours after the food reaches 70F in order to prevent bacterial growth. The beans failed to cool properly. COS: PIC voluntarily discarded the 2 large containers of refried beans. Corrected On-Site. New Violation. 6-2 Violation of Code: [511-6-1.04(6)(g)] Observed a pan of shredded chicken, a pan of cooked chicken breast, and a pan of black beans that the PIC states had been prepared 2 days ago. Time/temperature controlled for safety foods that are prepared and held in a food service establishment for longer than 24 hours must be clearly marked with a time/date stamp to indicate the time/date the food was prepared or the time/date the food is to be discarded. COS: PIC labeled the chicken and beans with the appropriate date. Corrected On-Site. New Violation.
