Violation of Code: [511-6-1.04(4)(c)1(iv)] Observed Cod in a reach-in cooler without any lid or cover. Food must be stored covered except when cooling in order to prevent overhead contamination. Violation from Base of Operation. COS: A lid was placed on the open container. New Violation. 4-2B Violation of Code: [511-6-1.05(6)(n)] Observed a Bleach Sanitizing Bucket located next to the 3-comaprtment sink with little to no sanitizer reading, and observed low temperature sanitizing dish machine with little to no chlorine sanitizing reading as well. Chemical sanitizers must be held at approved concentrations to be both safe and effective. Violation from Base of Operation. COS: New chlorine bucket was made with correct 100ppm. Work order will be placed to check low temperature sanitizing dish machine. New Violation.
