Violation of Code: [511-6-1.04(4)(c)1(iv)] Observed liquid margarine and bacon not in use out on counter without lids. All foods must be covered when not in use. COS: employee placed lids on both items. Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(7)(a)1] Observed icing nozzles and other equipment at icing station in back of kitchen with accumulations of food. Equipment food-contact surfaces and utensils shall be clean to sight and touch. COS: PIC cleaned nozzles with food accumulations. Corrected On-Site. New Violation. 6-2 Violation of Code: [511-6-1.04(6)(g)] Observed cooked chicken breast and cheese sauce with a discard date of 12/30/18. Time/temperature controlled for safety foods shall be labeled with a discard date not more than 7 days from opening/preparation of food item. COS: PIC stated items had been opened this morning and labeled the chicken and cheese sauce with the correct date label. Corrected On-Site. New Violation. 13A Violation of Code: [511-6-1.02(1)(d)] Observed inspection report posted from 2016 with a score of 98. Most recent inspection was from 5/15/2018 with a score of 80. The most recent inspection should be posted for public view in the food service establishment and the top 1/3 of the 1st page shall be posted in the drive-thru window. PIC states they do not have access to key for display case but will tape it to the outside with inspector present. Repeat Violation. 15A Violation of Code: [511-6-1.05(6)(a)] Observed meat freezer next to grill with lid not attached to freezer body. Heat from the grill has also destroyed the side of the lid, affecting the gasket. Equipment components such as doors, seals, and hinges shall be kept intact, tight, and adjusted. Discussed with PIC the need to replace freezer. Meat in freezer was 32F. New Violation. 15C Observed mixer head, under table shelves, sides of equipment, and floors with accumulated debris and residues. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust,dirt,food residue, and other debris. PIC to deep clean floors, equipment, and tables. New Violation.
