Violation of Code: [511-6-1.07(3)(a)] Observed no hand soap available in the men's restroom used by customers and employees. Each handwashing sink must be provided with hand cleanser for proper hand washing. COS: PIC refilled hand soap in men's restroom. Corrected On-Site. New Violation. 4-2A Violation of Code: [511-6-1.04(4)(c)1(iv)] Observed oil for pretzels and chocolate sauce stored on front food service countertops with no lid. All foods must be stored covered in between uses to prevent contamination. COS: PIC placed lids on both items. Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(7)(a)1] Observed grill food prep areas with build up of grease. Observed icing station with icing accumulated around nozzles and tray. Food-contact surfaces shall be clean to sight and touch. COS: PIC cleaned and sanitized grill areas and icing station. Corrected On-Site. New Violation. 6-2 Observed turkey in reach in cooler in grill area with a discard date of 1/2/19. Employee states turkey was prepared that morning and was not labeled correctly. Time/temperature controlled for safety foods that are prepared/opened and are held for longer than 24 hours shall be date/time labeled with a discard date/time not longer than 7 days. COS: PIC labeled turkey with correct date/time sticker. Corrected On-Site. New Violation. 8-2B Violation of Code: [511-6-1.07(6)(c)] Observed insecticide being stored next to sanitized dishes/utensils on storage rack. Observed insecticide being stored next to single service spoons and forks. Poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, and single service and single-use articles by separating the poisonous or toxic materials by spacing or partitioning and locating the poisonous or toxic materials in an area that is not above food, utensils, or single-service articles. COS: PIC moved both cans of insecticide to an approved designated storage area. Corrected On-Site. New Violation. 15C Violation of Code: [511-6-1.05(7)(a)2,3] Observed multiple nonfood-contact surfaces in kitchen with accumulations of food and grease debris. Surfaces include grill area cooler with a heavy accumulation of food debris, cake decorating area, and tortilla holders. Nonfood-contact surfaces shall be kept free of an accumulation of dust, dirt, food residue, and other debris. PIC to clean all nonfood-contact surfaces. New Violation. 17C Violation of Code: [511-6-1.07(5)(a),(b)] Observed grill freezer with damaged grate and an accumulation of food debris inside cooler body. Food debris was not in a food-contact area of freezer. Freezer is badly dented and not level. All physical facilities shall be maintained in good repair. Discussed with PIC the need to close off opening in bottom of freezer to prevent the accumulation of food debris and harborage of pests. New Violation.
