Violation of Code: [511-6-1.03(5)(j)1&2] Observed open bottle of aloe vera drink being stored next to food in dry storage closet. Employee beverages must be stored in designated areas. COS: PIC moved drink to designated area. Corrected On-Site. New Violation. 4-2A Violation of Code: [511-6-1.04(4)(c)1(iv)] Observed sliced tomatoes and lettuce being stored in walk-in cooler with no lid/wrapping. All foods must be stored covered when not in use. COS: PIC covered tomatoes and lettuce. Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(7)(a)1] Observed inside lid of preptop cooler with encrusted tomato sauce. Observed dough press with an excess of grease build up. Observed pizza pans with excessive grease build up. Observed microwave oven with heavy food residue. Food-contact surfaces shall be kept clean and sanitized when not in use. PIC to clean all food contact surfaces. New Violation. 6-2 Violation of Code: [511-6-1.04(6)(g)] Observed breaded cooked chicken, chicken wings, and tomato sauce PIC states had been prepared on Friday or Saturday 1/11 or 1/12. Time/temperature controlled for safety foods that are prepared and held for longer than 24 hours must be date/time labeled and discarded or used within seven days. COS: PIC labeled all foods that had been prepared over 24 hours ago. Corrected On-Site. New Violation. 15C Violation of Code: [511-6-1.05(7)(a)2,3] Observed work tables, floors, and walls with accumulated food debris. Nonfood-contact surfaces shall be clean to sight and touch. PIC to clean dirty floors, walls, and tables. New Violation.
