Violation of Code: [511-6-1.03(3)(c)] Observed no CFSM certificate posted in public view. CFSM states she was recertified in 2015 but recently moved stores and is in the process of obtaining a copy of the certificate. CFSM Amanda Stebel to provide a copy of the certificate to the health authority upon receipt. New Violation. 6-2 Violation of Code: [511-6-1.04(6)(g)] Observed boneless cooked chicken wings with a discard date of 12/7/18. Time/temperature controlled for safety foods that are commercially prepared shall be clearly marked at the time the original container is opened to indicate the day and time the food shall be consumed or discarded, when held at a temperature of 41F for a maximum of 7 days. COS: PIC voluntarily discarded chicken. Corrected On-Site. New Violation. 15C Violation of Code: [511-6-1.05(7)(d)] Observed computer keyboard next to preptop cooler with heavy accumulation of food debris. Nonfood-contact surfaces must be cleaned frequently enough so they do not accumulate soil residues. COS: PIC cleaned and sanitized keyboard thoroughly. Corrected On-Site. New Violation.
