Violation of Code: [511-6-1.03(2)(a)-(l)(n)(o)] PIC unable to: state correct temperatures for cold holding, hot holding, internal cooking for raw animal foods, or cooling, use a food probe thermometer correctly, and use test strips for chlorine sanitizer used in establishment. Upon beginning of inspection, food handling staff stopped working in food service areas until inspection concluded- PIC reported staff didn't want to make mistakes. The PIC should be able to recognize incorrect food handling practices and practice active managerial control to correct violations as they occur by using educational methods such as show/tell/show and leading by example so that correct food handling practices are consistent during daily operations. New Violation. 2-2D Violation of Code: [511-6-1.06(2)(o)] Server station handsink filled with dirty utensils pre-soaking in water. Side kitchen handsink blocked with stacked bucket lids. Handsinks must remain accessible at all times during operation and used for no other purpose than washing hands. COS- PIC removed utensils from sinks. Corrected On-Site. New Violation. 2-2D Violation of Code: [511-6-1.07(3)(a)] No soap available at server station and warewashing area handsinks. Soap must be provided at all handwashing stations. COS- PIC refilled empty dispensers with liquid soap. Corrected On-Site. New Violation. 2-2D Violation of Code: [511-6-1.07(3)(b)] No paper towels available at side kitchen and warewashing area handsinks. Hand drying provisions must be maintained at handwashing station areas. COS- PIC placed a paper towel rolls in clean containers and placed at both handsinks. PIC reported paper towels ordered did not fit dispensers and to purchase correct paper towel size for dispenser. Corrected On-Site. New Violation. 15A Violation of Code: [511-6-1.05(6)(a)] Dirty soft serve machine not being used because cannot hold correct temperature per PIC. Both makeline coolers have accumulated water pooling at bottom of units. The left side makeline cooler not holding TCS foods at time of inspection because unit does not hold correct temperature per PIC. Ambient temperature was 63F. Coolers must be maintained in good repair so operate according to manufacturers specifications. Coolers that are not properly repaired must be removed from the premises. PIC must also ensure adequate cold holding capacity is maintained at the establishment as broken coolers are being removed. New Violation. 18 Violation of Code: [511-6-1.07(5)(k)] Dead and alive roaches observed in side kitchen area. Many pipes, drains, and equipment found with leaking or standing water during inspection which may be contributing to pest harborage conditions. Health Authority discussed with PIC developing a treatment plan with their CPO to supplement current routine preventative treatments until evidence of pests is minimized and controlled. PIC could not furnish pest control records past the date 9/5/17 and reported manager had electric records. Establishment has 3 days to furnish pest records to the Health Authority. Repeat Violation.
