Violation of Code: [511-6-1.03(3)(c)] Establishment did not have a CSFM document posted for the manager who currently is the only one certified. The original CFSM certificate shall be posted in public view in each food service establishment. Repeat Violation. 17B Violation of Code: [511-6-1.06(5)(n)] Observed doors to both trash and recycling receptacles open. Trash receptacle doors and lids must be shut when not in use in order to prevent pests. Repeat Violation.
