Violation of Code: [511-6-1.04(4)(w)2] Observed kids in lunch line grabbing lip contact parts of plastic forks and spoons from a bulk supply tub in which they were stored. Consumer self-service operations for ready to eat foods shall be provided with suitable utensils or effective dispensing methods that protect food from contamination. COS: Forks and spoons placed lip contact portion down into dispenser cups. Corrected On-Site. New Violation.
