Violation of Code: [511-6-1.04(4)(c)1(iv)] Various containers of food in reach in, prep top, and walk in coolers not protected from overhead contamination. In walk in cooler, unwrapped foods were located under the compressor. Except when cooling, foods may not be stored unwrapped. COS: PIC and CFSM applied plastic wrapping to all containers without covers. Corrected On-Site. Repeat Violation. 6-2 Violation of Code: [511-6-1.04(6)(g)] Many pans/containers of ready to eat food in reach in, prep top, and walk in coolers not labeled with date of preparation. Per PIC, all foods were prepared or will be held for a period greater than 24 hours. Foods that will last more than 24 hours must have a date mark to notify a 7 day disposition date. COS: Foods in all coolers were properly labeled with date of preparation. Corrected On-Site. New Violation. 18 Violation of Code: [511-6-1.07(5)(k)] Live roach observed along oven. PIC was able to kill and remove it. Insects must be controlled by effective pest control measures and tight barriers to exterior. Corrected On-Site. New Violation. 18 Violation of Code: [511-6-1.07(2)(m)] Door screen no longer tightly sealed to the frame creating several openings to outside via crack beneath the exterior door. Screen doors must be fitted with either 16 mesh to 1 inch screens, properly designed with air curtains, or tightly sealed to the wall to prevent the entry of insects and rodents. PIC plans to repair screen in near future. Repeat Violation. Correct By: 12/15/2017
