Violation of Code: [511-6-1.05(6)(n)] Quat sanitizer dispenser behind 3 compartment sink not dispensing any sanitizer at time of inspection. Food contact surfaces must be sanitized after washing with an approved concentration of chemical santizer. (Quat solutions shall be maintained between 150-400PPM). COS: Maintenance in process of correcting dispenser and obtaining replacement parts. All dishes washed this morning must be resanitized. Corrected On-Site. New Violation. 6-1A Violation of Code: [511-6-1.04(6)(f)] Melons, boiled eggs, and cream cheese not under temperature control for the duration of breakfast service and were recorded at above 41F (see log). Time/temperature control for saftey foods must be cold held at 41F or below. Discussed procedures for establishment to create Time as a Public Health Control Policy in order to keep foods out of temp control for a maximum of 4 hours. In order to use policy, a written statement must be provided and kept on file stating which foods are to be held out of temperature control. Foods must be logged with the time they are removed from refrigeration, and also labeled with at least a 4 hour discard time, at which they must be thrown out. Establishment has 4 days 9/30/17 to create a TPHC policy and must provide documentation to Health Department. New Violation. 15B Violation of Code: [511-6-1.05(3)(h),(i)] Quat test strips provided behind 3 compartment sink labeled with an expiration date of 9/15/14. Sanitizers used on site must be able to be tested with an approved test kit. COS: Provided establishment with several quat strips until they can purchase more Corrected On-Site. New Violation. 17B Violation of Code: [511-6-1.06(5)(o),(p)] Dumpster drain plug broken and no longer effective. Outdoor receptacles must contain plugs in their drains. New Violation. 17B Violation of Code: [511-6-1.06(5)(n)] Half of lid missing to top of dumpster. Outdoor receptacles must be covered with tight fitting lids and doors. New Violation. 17B Violation of Code: [511-6-1.06(5)(o),(p)] Wooden pallets and planks stacked up around and behind dumpster. Enclosures for dumpsters must be maintained free of unnecessary items and clean. New Violation.
