Must post documentation to wall in public view New Violation. 4-2B Violation of Code: [511-6-1.05(6)(n)] Sanitizer spray bottle producing less than 100PPM of quaternary ammonium color change on test strips. Quat sanitizer must be used at concentrations of 150-400PPM to effectively reduce the number of microorganisms. COS: Sanitizer bottle remade with new solution, testing at 200PPM Corrected On-Site. New Violation. 14B Violation of Code: [511-6-1.05(10)(a)] Stack of plastic containers found wet-nested on wire rack behind 3 compartment sink. After cleaning utensils must be air dryed or adequately drained before contact with food. COS: PIC separated out containers individually to dry Corrected On-Site. New Violation. 18 Violation of Code: [511-6-1.07(5)(l)] Fly accumulation noticed within the facility around the three compartment sink. Trapped insects must be removed from the premises at a frequency that prevents their accumulation. Management is aware of problem and working with a pest control operator to manage issue. Repeat Violation.
