4-1A Violation of Code: [511-6-1.04(4)(n)1] Saw food handler with gloves grab raw hamburger patty and raw chicken filet out of freezer, put in fryer, then proceed to handle buns and sliced cheese for the sandwiches without changing gloves between tasks. If used, single-use gloves shall be only used for one task such as working with ready-to-eat food or with raw animal food, used for no other purpose, and discarded when damaged or soiled, or when interruptions occur in the operation. COS: CFSM informed, who then educated employees on proper glove changing. Employee washed hands and applied new gloves. Buns with cheese voluntarily discarded. Corrected On-Site. New Violation. 15A Violation of Code: [511-6-1.05(1)(a)] Dicer primarily used for onions top surface coming apart and blade surfaces bent out of place. Maintain food contact surfaces of equipment smooth and easily cleanable and avoid using equipment that is pitting, chipping, crazing, scratching, scoring, or distorted. COS: Management already ordered new dicer, current device no longer to be used. Corrected On-Site. New Violation. 15A Violation of Code: [511-6-1.05(6)(a)] Walk in freezer had many icicles and a large sheet of ice on the floor beneath far shelf. Maintain equipment in good repair and proper adjustment. Management aware of issue, curtain to be installed on entrance to freezer. Corrected On-Site. New Violation. 15C Violation of Code: [511-6-1.05(7)(a)2,3] Gaskets in several coolers, notably the shake machine, with a large accumulation of food debris. Keep nonfood contact surfaces of equipment free of dust, dirt, food residue accumulation. New Violation. 17B Violation of Code: [511-6-1.06(5)(o),(p)] Buildup of panels, metal siding, and other debris behind dumpsters in enclosure. A storage area for dumpsters need to be maintained free of unnecessary items. New Violation.
