4-2B Violation of Code: [511-6-1.05(7)(a)1] Pans stacked on drying rack beside 3 compartment sink had heavy grease and food residue. All food contact surfaces must be clean to sight and touch. COS- All pans on the rack were moved to be re-washed, rinsed and sanitized. Corrected On-Site. New Violation. 12B Violation of Code: [511-6-1.03(5)(i)] Employees with beards were not wearing hair restraints over the beards. Loose hair, including beards and mustaches longer than 1/2 inch, must be covered with a hair restraint. New Violation. 15A Violation of Code: [511-6-1.05(4)(a)1,2] Drying shelf was located next to the vegetable sink and above the vegetable sink drain. This is not the original approved location of the shelf. Equipment used for the storage of cleaned and sanitized eauipment, utensils, or single service articles may not be located near sources of contamination. COS- Drying shelf was moved to the exterior wall of the WI cooler. Corrected On-Site. New Violation. 17B Violation of Code: [511-6-1.06(5)(o),(p)] Receptacle area was littered with food debris and trash. Area must be maintained to prevent the attraction of pests and rodents. New Violation. 17C Heavy mold buildup at sink/wall juncture behind 3 compartment sink. Heavy mold/food buildup at covebase behind drying rack between vegetable sink and 3 compartment sink. Buildup of trash and food debris near covebase throughout establishment. Physical facilities must remain clean and in good repair. New Violation. Remarks New manager is taking CFSM course today. See attached verification email. Notification to be sent when employee receives certification.
