4-2B Violation of Code: [511-6-1.05(6)(n)] Spray bottle of quat sanitizer used on prep surfaces not registering any quat. Quat concentration must be kept between 100ppm and 400ppm for its' safe and effective use as a sanitizer. COS: CFSM refilled spray bottle from quat dispenser Corrected On-Site. New Violation. 6-1A Violation of Code: [511-6-1.04(6)(f)] Top of rear makeline cooler not holding chicken wings(44F) or chicken nuggets(44F) below 41F. Tomatoes in three compartment sink sitting out at 63F. Temperature of TCS foods must be kept at 41 degrees or below to limit microbial growth. COS: Tomatoes voluntarily discarded. Wings and nuggets, out less than 4 hours, were moved to walk in cooler. Corrected On-Site. New Violation. 13A Violation of Code: [511-6-1.02(1)(d)] Inspection posted in rear hallway in food preparation area off limits to customers. The inspection report must be displayed so that it is easily visible to patrons at all times where it can be read at a distance of one foot away. New Violation. 15B Violation of Code: [511-6-1.05(6)(f)] Middle compartment of three compartment sink found to contain colander full of diced tomatoes. A warewashing sink may not be used for preparing raw fruits/vegetables. COS: Tomatoes voluntarily discarded. Corrected On-Site. New Violation. 17A Violation of Code: [511-6-1.07(2)(l)] There is no self closer on employee restroom. Install self closer to meet requirements for restroom doors. CFSM stated one will be put on. Repeat Violation. 17A Violation of Code: [511-6-1.06(5)(h)] No lid on trash can in the employee restroom. Restrooms that are used by females must have a lid/cover for sanitary napkins. New Violation. 17B Violation of Code: [511-6-1.06(5)(n)] Lids to dumpster open. Dumpster lids must be closed to prevent the entrance of pests. COS: Lids flipped close. Corrected On-Site. New Violation. 18 Violation of Code: [511-6-1.07(2)(m)] Kitchen door found to be left sitting open with interior uncovered upon arrival for inspection. Outer openings of a food service establishment must be protected against entry of insects and rodents if left open through use of 16 mesh to 1 inch screen, properly designed and installed air curtains to control flying insects, or other effective means.
