4-1A Violation of Code: [511-6-1.04(4)(c)] Raw beef was stored on top of a bucket of soy sauce in the WI cooler. Unwashed vegetables were stored above RTE products (noodles, sauces, salad). Raw products must be separated from ready-to-eat products and unwashed produce must be segregated to prevent cross contamination. COS- Beef was removed, unwashed vegetables were segregated. Corrected On-Site. New Violation. 8-2B Violation of Code: [511-6-1.07(6)(p)] Pills stored in plastic bag in refrigerator on top of sauces. Pills stored in container on bottom shelf of refrigerator. Medicines belonging to employees stored in the refrigerator must be stored in a package or container and kept inside a covered, leak proof container that is identified as a container for the storage of medicines. COS- Medicines removed from refrigerator. Corrected On-Site. New Violation. 11D Violation of Code: [511-6-1.05(2)(k)] Only thermometers available were the cold holding unit ambient temp measuring devices. A thermometer for measuring cooking temperature, reheating temperature, hot holding temperature, cooling and cold holding temperature for foods must be provided. New Violation. 14A Violation of Code: [511-6-1.04(4)(k)] Plastic scoops with no handle were stored in flour, sauces, rice, etc. Scoops must have a handle and handle must be above product surface. COS- Scoops were discarded and replaced with scoops with handles. Corrected On-Site. Repeat Violation. 17D Violation of Code: [511-6-1.07(4)(b)] Toothbrush, toothpaste, employee beverage, and cell phones were stored on the prep counter. Store items in an area away from food preparation and food service. COS- Personal items were removed. Corrected On-Site. New Violation.
