Violation of Code: [511-6-1.03(3)(c)] Certified Food Safety Manager does not have certification documentation (certificate) posted. The original CFSM certification must be posted in public view New Violation. 4-2B Violation of Code: [511-6-1.05(7)(a)1] Slicer with accumulation of food debris on knife holder blade that was probably slung from blade. Slicer was not used today. After use prescrape slicer blade and all parts of slicer, wash, rinse & sanitize and recheck for cleanliness before storage and next use New Violation. 4-2B Violation of Code: [511-6-1.05(7)(b)] Can opener with large accumulation of food debris on blade and with large accumulation of metal shavings behind blade. After use, can opener should be washed, rinsed and sanitized and rechecked for cleanliness before storage + next use. New Violation. 6-2 Violation of Code: [511-6-1.04(6)(h)] Refried beans in walk in cooler dated 12/29, past 7 day use or discard date. Potentially hazardous food items prepared inhouse must be used or discarded within 7 days and the day of preparation is considered as day one. COS: Voluntarily discarded the refried beans in walk in cooler dated 12/29/16 Corrected On-Site. New Violation. 6-2 Violation of Code: [511-6-1.04(6)(g)] Pork cooling from prep and rapid cooling in blast chiller this AM sated as being prepared yesterday. Potentially hazardous food items made inhouse must be used or discarded within 7 days and the day of preparation is considered day one. COS: Pork properly dated with today's date as day one. Corrected On-Site. New Violation. 15A Violation of Code: [511-6-1.05(6)(a)] Can opener blade with large accumulation of metal shavings which can indicate blade is dull and needs to be replaced. Blade also appears to have a chunk of the metal missing. Needs replacing. New Violation.
