EVANS, Ga.--(BUSINESS WIRE)--Sep 12, 2017--PPCN and Creative Pharmacist today announces a new partnership that will improve the health of all Pennsylvanians. PPCN has selected Creative Pharmacist’s STRAND platform to help their member pharmacists provide comprehensive medication management services to the patients in the communities they serve. PPCN is a newly formed network of high quality community pharmacies with specially trained pharmacists in Pennsylvania that have joined together to be able to demonstrate the value local, community-based pharmacists bring to the healthcare system by providing clinical services for their patients.
STRAND is a first-of-its-kind intervention platform, allowing pharmacists to create and transmit Pharmacist eCare plans and bill for clinical services. In addition, STRAND provides intervention support, including patient education, built directly into the platform, allowing pharmacists to engage patients through an easy-to-use pathway within their workflow. STRAND also has single sign-on capabilities with a majority of pharmacy management systems.
“PPCN is thrilled to be working with Creative Pharmacist as we launch our first payer contract,” said Mindy Kozminski, executive manager of PPCN, Senior Program Manager, University of Pittsburgh School of Pharmacy. “Our pharmacists are poised to make a positive impact on the patients they care for and Creative Pharmacist’s STRAND provides the pharmacists with a robust software to not only document, but provide tools and resources for patients. It is an ideal partnership.”
“Today’s healthcare landscape is full of challenges and opportunities for pharmacists. Pharmacists are ready to elevate the level of care they provide for their patients, but need a clinical services platform that keeps pace with rapid changes in the industry,” said David Pope, PharmD, CDE, chief of innovation and co-founder of Creative Pharmacist. “The STRAND intervention platform is that technology. We are excited about our partnership with PPCN and look forward to providing a clinical resource for pharmacists in their network to use as they engage their patients.”
Through this partnership, PPCN and Creative Pharmacist will be able to provide pharmacists with a technology to document care and capture outcomes that demonstrate the value community pharmacists bring to patients and the healthcare community.
