The Pennsylvania Pharmacists Care Network (PPCN) and Creative Pharmacist Launch Partnership to Improve Patient Health

EVANS, Ga.--(BUSINESS WIRE)--Sep 12, 2017--PPCN and Creative Pharmacist today announces a new partnership that will improve the health of all Pennsylvanians. PPCN has selected Creative Pharmacist’s STRAND platform to help their member pharmacists provide comprehensive medication management services to the patients in the communities they serve. PPCN is a newly formed network of high quality community pharmacies with specially trained pharmacists in Pennsylvania that have joined together to be able to demonstrate the value local, community-based pharmacists bring to the healthcare system by providing clinical services for their patients.

STRAND is a first-of-its-kind intervention platform, allowing pharmacists to create and transmit Pharmacist eCare plans and bill for clinical services. In addition, STRAND provides intervention support, including patient education, built directly into the platform, allowing pharmacists to engage patients through an easy-to-use pathway within their workflow. STRAND also has single sign-on capabilities with a majority of pharmacy management systems.
